Anyone arriving in Ireland in June 2020 had to complete a form like this. Introduced on 24 April 2020 the form recorded email, place of residence, mode of transport and passport details. Everyone arriving into Ireland was also expected to completely self-isolate for 14 days to minimise the risk of spreading COVID-19. The form meant that checks could be made to ensure people were isolating and for contact tracing. Contact tracing is the process of identifying people who may have come into contact with someone with COVID-19 in order to limit the spread of the virus.
In August 2020, a digital version of the form was later introduced and later included vaccine declarations and COVID-19 tests. Quarantine rules changed during the pandemic depending on which country you were arriving from.
