The form includes a list of schools masters, timetable of subjects and separate fees for "Students of "Industrial Classes" and "Middle Classes". On the back a declaration to be filled out by students of industrial classes in which they promise to attend at least 22 lessons or get charged the fees of a Middle-class student.
