the first [struck through] Waggon of the train came in contact with the throw off switch and broke 3 Wheels and other damages to the amount of 6 Pounds or upwards and it is John Craggses duty to attend the Switch and see that all are Right before the Waggons Came dowen. {The switch new put in, neglect not evident}
January 24th the switches at the high end of the long siding above Urley nook was out of order, the way men knew that they were so; and did not attend to them; When the Majestic Engine came dowen one of the switches was out of its place, and it was with great Dificulty the Engine man and his Assistant saved the Engine from being throwing off the way

into the Ditch I told John Bell to bring there names and reasons they assigne for not attending to the switches and likewise the switches below Urley nook that was named here last week as being 3 times wrong dureing the last Week. {Geo Hall & John Thompson}
